The Supportive Housing and Health Initiative (SHHI)

(SHHI) is a comprehensive program, designed to assist tenants residing in both fixed and scattered supportive housing sites. The initiative aims to provide a holistic approach to care, recognizing the significant link between housing and health outcomes. The program is specifically tailored to support individuals transitioning from chronic homelessness to supportive housing, a change that, while beneficial, can present numerous challenges.

The main goal of SHHI is to facilitate tenants’ successful adaptation in their new living environments by providing multi-disciplinary support including system navigation, primary care access or support, nursing care, and specialized counselling.

Tenants seeking to engage with the SHHI program can connect on-site at supportive housing locations and individuals located at a scattered site can access our drop-in clinic Wednesdays 1-4PM at Sanguen’s Kitchener location (130 Victoria St. S).
